Does our Benefit Plan cover custom-made orthotics?

If you qualify for Benefit Levels 02, 03, or 04, you may be eligibile to claim custom-made orthotics. Please note, your custom-made orthotics must be prescribed by a licensed podiatrist, chiropodist, chiropractor, or physician; limited to a maximum of $450 per individual every two plan years.

Orthopaedic Shoes: when not attached to or forming part of a brace, to a maximum of $300 per individual every plan year.

Orthopaedic Applicances: Devices and SHoes (when attached to and form part of a brace) when prescribed to a maximum of $450 per individual every plan year.

NOTE: When you purchase custom-made orthotics, shoes, appliances and/or devices, they must be purchased through a supplier who has a specialty in orthotics, as they are a custom fitted and manufactured item. Please contact Canada Life prior to purchasing your orthotics to ensure your prescription and the place where it is filled meets the necessary requirements.
