SPENDING BENEFIT
Available Account:
- Level 02 with $1,300 Health Spending Benefit
- Level 03 with $1,500 Health Spending Benefit
- Level 04 with $1,500, $2,500 or
$3,500 Health Spending Benefit
PLEASE NOTE:
- Account Balances from Producer
contributions between $7,450.97 to $10,434.61 will have a $1,500.00
health spending benefit
- Account Balances from Producer
contributions between $10,434.62 to $15,401.92 will have a $2,500.00
health spending benefit
- Account Balances from Producer
contributions over $15,401.93 will have a $3,500.00 health spending
benefit
- Age 75 and over: Producer contribution balances up to a maximum of
$3,967.31 will be placed in a health spending benefit and can be used for
reimbursement of any qualifying dental or medical expense.
Payment
from Health Spending Benefit:
- co-insurance,
- amounts which
exceed Plan maximums for health and dental, and
- other eligible
health expenses the Plan does not usually pay, but are allowable under
the Income Tax Act and Provincial Legislation requirements
Expenses from the April 1 2010 to March 31 2011 plan year MUST BE submitted
to Manulife by no later than September 30
2011. IF you have any unused health spending money left over it will
be carried over to the new plan year, HOWEVER, your claims will not,
therefore it is imperative your claims are submitted by the deadline.
The chart below is designed to help you determine when your claims must be
filed.

Any
unused portion of the Member’s Maximum Benefit remaining at the end of
the Plan Year will be carried forward to be used in the following Plan
Year. However, if the amount carried forward is not used by the end of the
following Plan Year, it will be forfeited.
Payment Election: each member elects which claims are to be paid from the
Health Spending Benefit (see the health and dental claim forms on the Download
Forms page).
The
Health Spending Benefit described on this site is not insured but is
administered by Manulife and is payable from the funds allocated to
your Health Spending Benefit.
