| BENEFIT
SUMMARY
Plan members and their dependents are eligible to be insured for the following benefits for the plan year April 1, 2007 to March 31, 2008. If you were a member on April 1st, you received correspondence confirming your benefit level. If you are a new member, you are eligible for benefit level 01. You may pay the cost of upgrading to benefit level 02. You may pay the cost of upgrading to benefit level 02 ninety (90) days after you become a plan member. Each year the Board of Trustees will decide upon the benefits that members and their eligible dependents will receive for the 12 month plan year starting April 1st. You will receive benefit coverage under this Plan as long as you are a member in good standing of I.A.T.S.E. Local 667 or Local 669. Your benefits will be based on the contribution balance and pay direct payments you make to the Plan. BENFIT

DENTAL BENEFIT Benefit levels 02, 03, 04 for plan members & eligible family members. The dental care benefit includes basic and restorative services as covered expenses. Benefit levels 04 (default) & benefit level 04 with health spending benefit also includes major services & orthodontics (eligible dependants under the age of 19) as covered expenses. MEDICAL BENEFIT Benefit levels 02, 03, 04 for plan members & eligible family members. The medical benefit includes the following benefits as eligible expenses:
If you are a Canadian resident and incur any of the above eligible medical care expenses outside your province of residence, reimbursement will be based on the reasonable and customary charges for the area in which they are incurred. The provincial medicare plan in your province of residence must reimburse any eligible expenses prior to reimbursement under the I.A.T.S.E. Health & Welfare Benefit Plan. Please remember, in order to maintain eligibility for coverage, a Plan Member must maintain membership in good standing in I.A.T.S.E., Local 667 or Local 669 and subject to any age and residency requirements. PLAN MEMBERS WHO RESIDE OUTSIDE OF CANADA If you and your dependents are eligible to receive Benefit Level 01 coverage but you are a Plan Member who resides outside of Canada, your coverage includes: Life Insurance If you and your dependents are eligible to receive Benefit Level 02, 03, or 04 coverage but you are a Plan Member who resides outside of Canada, your coverage includes:
