SCHEDULE OF LOSSES
If, as a direct result of an accident, while covered, you suffer one of the losses specified below, the following benefits are payable, (if more than one of the benefits could be payable for injuries resulting from the same accident, only the greatest benefit applicable will be payable for the loss):

Loss of the sight of an eye means the total and irrecoverable loss of the entire sight thereof. Loss of an arm or leg means actual severance through or above the elbow or knee joint respectively. Loss of a hand or a foot means severance at or above the wrist or ankle joint respectively. Loss of a thumb and index finger or fingers and toes means severance at or above the metacarpophalangeal joints.

Loss of speech or hearing means the total and irrecoverable loss of speech or hearing and, without limiting the foregoing, such loss must have been continuous for a period of at least 12 consecutive months.

Loss of use means the total and irrecoverable loss of use and, without limiting the foregoing, such loss must have been continuous for 12 consecutive months.

Quadriplegia means the total and irreversible paralysis of both upper and lower limbs.

Paraplegia means the total and irreversible paralysis of both lower limbs.

Hemiplegia means the total and irreversible paralysis of upper and lower limbs of one side of the body.

Paralysis must be continuous for a period of at least 12 consecutive months, after which the benefit is payable provided the disability is determined to be permanent.

Manulife may require that any "Loss" or "Loss of Use" or "Paralysis" described above be confirmed by a licensed physician.

The loss must occur within 365 days of the accident. If you sustain more than one loss as a result of the same accident, then payment will not exceed 100% of the amount shown in the Schedule of Losses except in the event of benefit payment due to Quadriplegia, Paraplegia, or Hemiplegia, in which case such amount will not exceed 200% of the amount shown in the Schedule of Losses.

Exposure and Disappearance

If as a direct result of an accident, you suffer a loss due to exposure to the elements of nature while insured, Manulife will pay the amount provided for that loss based on the provisions of the Plan in effect on the date of the accident.

If you disappear while insured as a direct result of the accidental sinking, wrecking or disappearance of a vehicle in which you were an occupant, and your body is not found within one year thereafter, then Manulife will pay, on the presumption of death, the amount provided for loss in accordance with the provisions of the Policy in effect on the date of your disappearance.

Excluded Losses

No payment will be made for any loss caused by:

- suicide or self-inflicted injuries,

- insurrection, war, riot or civil commotion,

- committing or attempting to commit a criminal offence,

- bodily or mental infirmity or disease,

- infection unless as a direct result of an accidental wound or cut,

- voluntarily taking or inhaling any poison, drug or gas,

- travel or flight in, or descent from any device for aerial navigation or any air travel, if you are a pilot or other member of the crew of the aircraft or if such flight is made for purposes of instruction, training, or testing.

Repatriation

If an AD&D benefit is paid for the loss of your life, while insured, as a direct result of an accident occurring at least 200 km from your permanent city or town of residence, a repatriation benefit may also be paid under this Plan.

The repatriation benefit provides up to a maximum of $10,000 for the reimbursement of actual expenses incurred (which are not payable under any other benefit of this booklet) to prepare your body for burial or cremation. Transportation of your body to your ordinary place of residence in Canada is also included, when this differs from the place where death occurs.

If any other benefit in this booklet, or any other policy with Manulife provides similar coverage for repatriation, the other benefit pays first. In no event will the total amount payable under this Plan, and any other policy with Manulife exceed 100% of the actual expenses incurred.

Rehabilitation

If an AD&D benefit is paid because you suffer a loss (other than loss of life) while insured, as a direct result of an accident, a rehabilitation benefit may also be available from the Plan.

The rehabilitation benefit provides up to a maximum benefit of $10,000 for repayment of reasonable and necessary expenses incurred by you for special training, provided that:

1. the training is required as direct result of such loss, and training is reasonably necessary for you to be able to perform an occupation, that you would not have been engaged in, except as a result of such loss;

2. the expenses with respect to such training, are incurred within 2 years from the date of the accident; and

3. the expenses are not for room, board or other ordinary living, travelling or clothing expenses.

If any other benefit in this booklet, or any other policy with Manulife provides similar coverage for rehabilitation, the other benefit pays first. In no event will the total amount payable under this Plan, and any other policy with Manulife exceed 100% of the actual expenses incurred.

Spousal Occupational Training Benefit

If an AD&D benefit is paid because a Member dies while insured, as a direct result of an accident, a spousal occupational training benefit may also be available from the Plan.

The spousal occupational training benefit provides up to a maximum of $5,000 for the repayment of reasonable and necessary expenses incurred by the spouse of the deceased member for special training, provided that:

1. the training for such spouse is required as a direct result of such loss, and such training is reasonably necessary for the spouse to be able to perform an occupation, for which he/she would not have been engaged in, except as a result of such loss;

2. the expenses with respect to such training, are incurred within 2 years from the date of the member's death; and

3. the expenses are not for room, board or other ordinary living, travelling or clothing expenses.

If any other benefit in this booklet or any other policy with Manulife provides similar coverage for spousal occupational training, the other benefit pays first. In no event will the total amount payable under this Plan, and any other policy with Manulife exceed 100% of the actual expenses incurred.
